Chest tattoos for women: placement and movement
A chest tattoo on a woman has to be planned around tissue that moves, which is why artists anchor the design to the bone underneath it.
- Pain level7 to 8 / 10 on sternum
- Sternum panel10 to 15 cm wide
- Healing3 weeks, no underwire
- Studio cost$300 to $1,200

The chest is several placements at once. The shelf under the collarbones, the sternum panel running down the center, the upper slope of the breast, and the crease underneath it. They share a session and a bra, and they behave completely differently once healed, because some of them sit on bone and some sit on tissue that changes size.
That difference decides most of the design. Bone is a fixed reference: the sternum, the collarbones and the ribs at the outer edge do not move, no matter what your body does. Breast tissue moves with weight, with pregnancy and with time. A design that puts its structure on bone and its soft elements on soft tissue holds its shape for decades.
Anchoring a design so movement does not break it
Breast tissue changes volume with weight, with the menstrual cycle to a small degree, with pregnancy and breastfeeding a great deal, and with age over decades. Ink in that tissue moves with it. A face, a clock, a geometric grid or anything the eye checks for accuracy shows distortion quickly, because your brain knows what those shapes should look like.
Artists get around this by running the load bearing parts of the composition over the sternum, the collarbones and the rib edge, then letting petals, leaves, feathers and ornamental flourishes drift onto the soft tissue. Those elements can shift a few millimeters over ten years and still look correct. Ask your artist which parts of your design sit on bone before you agree to the stencil.
What holds up on moving tissue and what does not
- Flowing botanicals and ornamental shapes tolerate movement well
- Evenly spaced geometry and mandalas reveal distortion first
- Portraits and clock faces are the least forgiving of all
- Lettering across the breast can bow out of line, lettering on the sternum stays straight
Sternum, underbust and the symmetry problem
The sternum gives you a panel roughly 10 to 15 cm wide and 15 to 25 cm tall between the collarbones and the bottom of the ribcage. It is the only true vertical axis on the chest. Underbust designs sit in the crease and follow it outward, usually 20 to 30 cm across in total.
Very few people are symmetrical, and breasts are rarely the same size or sitting at the same height. A mirrored design across the sternum will make that difference visible in a way you may not have noticed before. Good artists draw the two halves separately rather than flipping one, and check the stencil with you standing, then lying down, because the underbust crease moves several centimeters between those two positions.
Pain, session length and what to expect on the day
The sternum is 7 to 8 out of 10. Bone sits directly under thin skin and the whole area moves with every breath, which is the same problem the ribs have. The collarbone rates about 7. The soft upper slope of the breast is easier, around 5, and the closer the work comes to the nipple the sharper it gets. Many artists will not tattoo the areola at all.
Plan for two to four hours and expect to be tattooed in a reclined position wearing a disposable cover or your own strapless top. Ask the studio in advance how they handle draping if that matters to you. Sternum work is often split across two sittings, because tolerance on bone drops steeply after the third hour.
Healing under a bra, and when to wait
Underwire sits exactly where an underbust tattoo goes, and a band rubs a sternum piece at its lowest point. For two to three weeks you need a soft bralette, a front opening sports style with no wire, or nothing at all. Sweat collects in the crease under the bust, so keep the area dry and change what you wear against it daily. This is the most common reason underbust work heals patchy.
If you are pregnant or planning to be within a year or two, most artists suggest waiting on chest work. Breasts often grow one to two cup sizes in pregnancy and change again after feeding stops, and the skin over them can stretch enough to leave permanent marks through a tattoo. The same conversation applies before a planned reduction, augmentation or surgery, since a healed design cannot be moved with the tissue.

Questions people ask
Will a chest tattoo change shape if my body changes?
Ink sitting on breast tissue moves with that tissue, so weight change, pregnancy and time all shift it. Designs anchored on the sternum, collarbone and rib edge stay put. Ask which parts of your stencil sit on bone. Flowing shapes tolerate the movement, geometry and portraits do not.
Should I wait until after pregnancy for a chest tattoo?
Most artists advise it if pregnancy is likely in the next year or two. Breasts commonly grow one to two cup sizes and change again after feeding stops, and the skin can stretch permanently. Work anchored high on the collarbone is the least affected, so it is the usual compromise.
How painful is a sternum tattoo?
Around 7 to 8 out of 10. Thin skin sits directly on bone, and the area rises and falls with every breath so you cannot hold still. The collarbone is similar. The soft upper chest is easier at about 5. Most artists split larger sternum pieces over two sessions.
What do I wear while an underboob tattoo heals?
No underwire for two to three weeks. An underwire sits exactly on the design and a band rubs the lowest part of a sternum piece. A soft bralette, a wireless front opening sports style or nothing works. Sweat gathers in the crease, so keep it dry and change what you wear daily.
Can a chest tattoo be symmetrical?
It can be drawn symmetrically around the sternum, but very few bodies are symmetrical and a mirrored design makes any difference in breast size or height more visible. Artists who work here often draw each half separately by eye instead of flipping one, and check the stencil both standing and lying down.
